Registered Nurse - Triage

Health Partners North Suburban Family Physicians has an exciting opportunity for a Triage RN to join our team! This .80 fte position will be based at our Lino Lakes clinic with potential for coverage at our Hugo or Roseville clinics.

The schedule for this position will be Monday, Tuesday, Wednesday, Friday either 8:00am - 4:30pm or 8:30am - 5pm.

This position provides triage support for our clinic at the North Suburban Family Physicians clinics. Nurses are expected and supported to work at the top of your license by using protocols embedded in the electronic medical record and a variety of resources including Standing Orders and protocols. This position also includes occasionally working with patients that have questions or needs once on-site.

In addition, this role provides comprehensive assessments to establish and evaluate a plan of care and address changes in a patient's condition, delegates nursing tasks when appropriate and provides supervision and monitoring of function or activity, creates and implements dynamic teaching plans based upon identified patient need and acts as a resource for other staff.

Nursing Process:

Performance is up to standard when:

• Nursing care demonstrates clinical competence in using the nursing process to provide care to patients and their families, implements established Nursing Policies and Procedures and Standing Orders and provides emotional, intellectual and psychological support to patients and their families.
• Documents accurately and completely and keeps record regarding clinical practice.
• Consultation/collaboration: shares information with other members of the health care team, makes appropriate patient referrals, uses interdisciplinary resources to enhance own clinical decision making, provides continuity among home, office, inpatient setting and community, uses appropriate systems, automated or manual, to obtain patient information or schedule appointments.

Care Coordination:

Care Coordination is the ongoing support of patients and their families over time as they navigate care and relationships. It assists patients/families to optimally manage their health care, respect their culture and values and helps to identify individual needs, health goals and treatment preferences.

Performance is up to standard when:

• Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans
• Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care
• Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention
• Identifies and secures patient services needed for patient care
• Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.

Education:

Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding. It includes interaction with the patient, family and community, as well as participating in formal and informal professional education,
