Schedule: Part‑Time to Start + Weekend, on‑call, and holiday stipulations

Location: Greater Twin Cities Metro Area, MN (in‑home field‑based position)

• Provide direct clinical care including symptom and pain management
  , vital signs, medication monitoring, and wound care.
• Collaborate with the interdisciplinary care team—physicians, social workers, therapists—to develop and implement personalized care plans.
• Offer emotional support and education to patients and families navigating end‑of‑life care.
• Maintain accurate and timely documentation in compliance with hospice standards.
• Be available 24/7 for on‑call needs
  , including nights, weekends, and holidays, to ensure seamless patient support.

• Active LPN license in Minnesota.
• Prior experience in hospice, palliative, nursing home, or home health settings is required
  .
• Strong clinical judgment paired with empathy, patience, and excellent communication skills.
• Comfortable working autonomously in patients’ homes and coordinating care across disciplines.
