We are seeking passionate Service Coordinators who are committed to community advocacy and the wellbeing of underserved populations.

Position Summary

Provide case management services to individuals with intellectual and/or developmental disabilities, ensuring the delivery of high‑quality, person‑centered care that meets state standards.

Core Responsibilities
  • Advocate on behalf of individuals, provide resource information, and educate families and the community to promote self‑determination.
  • Collect, assess, and document documentation related to eligibility for DDA services; complete Comprehensive Assessment (CA) and Person‑Centered Plans (PCPs); write Self‑Directed Plans.
  • Coordinate and facilitate service referrals, liaise with providers, and monitor service delivery, including reporting on outcomes and recertification (RECON).
  • Travel to meet with support staff, conduct on‑site visits, and maintain HIPAA‑secure records.
  • Collaborate in team meetings, leadership reviews, and policy compliance activities.
  • Ensure all activities meet health‑care confidentiality, HIPAA, and state‑mandated regulatory requirements.

Education Requirements
  • Bachelor’s degree in an accredited human services program.
  • Or bachelor’s degree in a non‑human services field with two years of experience in a human services role.
Preferred Experience

Experience working with people who have developmental disabilities.

Physical Demands

Standard office environment; may occasionally move office equipment weighing 10–25 pounds; requires driving and computer operation.
