We have an exciting opportunity available for an enthusiastic and highly motivated individual to join our Community Mental Health Team. The role focuses on delivering high‑quality and effective services to people with severe and enduring mental health problems, promoting independence and recovery.

This position is open only to current employees of the following NHS organisations:

• Cornwall Foundation Trust
• Royal Cornwall Hospitals NHS Trust
• Cornwall and Isles of Scilly Integrated Care Board

If you are not a current employee of one of these organisations, your application will not be considered.

• Assess care needs and clinically manage a defined caseload within the Cornwall Partnership NHS Foundation Trust mental health service.
• Develop, implement, and evaluate programmes of care using the care co‑ordination system.
• Develop the interface between primary and secondary care to offer focused, co‑ordinated packages of care appropriate to patient needs.
• Demonstrate knowledge of the CPA process, mental health legislation, the Mental Health Act, and general mental health issues.
• Work collaboratively and flexibly as part of a multidisciplinary team, contributing to service innovation and development.

This is a community‑based role that requires frequent travel to various locations throughout the county, including some areas not served by public transportation. Candidates must be able to demonstrate how they will manage these travel requirements.

• Current registration with the Nursing and Midwifery Council (NMC).
• Completed Post Basic Training.

• Experience working with adults with enduring mental illness.
• Community nursing experience.
• Knowledge of electronic record systems, care co‑ordination, risk assessment, management, and suicide awareness.
