Caradon Community Mental Health Team are seeking an experienced and dedicated Senior Community Mental Health Nurse to join our dynamic team within the Cornwall Partnership NHS Foundation Trust Mental Health Service. • Significant experience in mental health services and care coordination.
• Strong communication and organisational skills, with the ability to work collaboratively across multi-disciplinary teams.

This is a community-based role that may require frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

• Toberesponsiblefortheassessmentofcareneedsandclinicalmanagementofadefinedcaseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
• Tocarrycontinuingresponsibilityforthedevelopment,implementationandevaluationofprograms of care, utilising the care co-ordination system.

• Todeveloptheinterfacebetweenprimaryandsecondarycaresservicessothepatients/carersare offered focused and co-ordinated packages of care that are appropriate to their needs.
