Responsibilities
  • Collaborates with physicians and mid-level providers and recommends adjustments to drug therapy based on clinical status and treatment outcomes
  • Coordinates with clinical team to ensure all aspects of pharmaceutical care are delivered in a safe and timely manner
  • Responsible for supervising pharmacy technicians, coordinating workflow and acts as resource for other infusion sites as needed
  • Participates in clinical research, education, professional development and performance improvement
  • Requires independent decision making, clinical judgement and in-depth knowledge of oncology and therapeutics
ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Enter medication orders into the computer system and dispense medications/IV solutions accurately and in a timely manner
  • Critically evaluate medication therapy and make recommendations to improve patient medication regimens, documenting patient assessment data and clinical interventions
  • Ensure appropriate and cost-effective drug therapy, addressing drug-drug interactions, contraindicated medications, inappropriate therapeutic regimens, adverse drug reactions, pharmacokinetics, disease state management, and rational effective medication regimens according to evidence-based guidelines
  • Function as a therapeutic consultant and serve as a resource and drug information specialist to medical, pharmacy, nursing staff and allied healthcare providers
  • Conduct educational activities, apply principles of performance improvement, perform comprehensive clinical assessments and collaborate with the integrated healthcare team to provide quality patient care
  • Apply principles of continuous quality improvement consistent with job expectations, incorporating quality improvement principles into other activities and projects (e.g., data collection, documentation), developing criteria for drug use evaluations, medication use indicators and disease-based evaluations
  • Provide oversight and assist with the development and implementation of evidence-based medication guidelines/protocols, designing and implementing medication monitoring guidelines and programs for clinical staff
  • Assist the director of pharmacy with ensuring compliance with state and federal laws, validation of chemotherapy protocols and therapy plans in the EMR, and maintaining competencies required for oncology practice
  • Oversee activities of pharmacy technicians, coordinate workflow and perform final check of medications prior to being dispensed, ensuring compliance with clean room policies and procedures in accordance with USP 797 and 800 guidelines
  • Provide counseling and drug education to patients and families, including medication therapy management, and participate in staff education via educational in-services, presentations, medical literature searches, and individual counseling
  • Participate in inventory control including purchasing, receiving and stocking medication and pharmacy supplies
  • Comply with governmental and other regulations related to preparation, packaging and storage of products
