The Home Health Aide (Medicare Certified) will provide home health patient/resident care and perform various treatments under the direction of the licensed nurse as required. Completing assigned tasks to ensure timely, efficient and safe patient/resident care. Communication and interactions with patients, residents, families and other health team members is an essential component of this position.

• Performs initial and ongoing data collection on home health patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
• Implements plan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural, ethnic and religious/spiritual needs.
• Provides direct patient/resident care and reports condition or status changes accurately and timely to the RN or LPN.
• Demonstrates time management and prioritization of tasks.
• Documents results of patient/resident care within established policies, procedures and scope of practice.
• Follows facility policies on medication administration and treatments under the direction of a licensed nurse.
• Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company.
• Performs other duties, tasks and/or projects as assigned.

QUALIFICATIONS REQUIRED

• Must be in good standing on the State Nursing Assistant registry or appropriate entity
• Valid Driver's License
• Acceptable driving record that meets Benedictine requirements.

QUALIFICATIONS PREFERRED

• Previous home health, hospital, or nursing home experience preferred
• BLS Certification.
