Outpatient Physical Therapist

• Monday through Friday schedule with 2-3 evenings per week until 7:00 PM
• 40 hours per week guaranteed
• Day and hours are negotiable
• 13-week assignment
• 1 year of outpatient clinical experience required
• Degree in Physical Therapy from an accredited school required
• Active and applicable state license required for the state where work will be performed
• CPR certification required
• Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Responsible for delegation and direction of all discipline-specific therapy services for assigned patients
• Design functional treatment programs to support effective return to community activities
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry
• Ensure care is provided under signed, valid physician orders and within insurance approval
• Participate in student programs as assigned, providing appropriate supervision and completing student evaluations
• Adhere to discipline-specific State Practice Act and all other applicable regulatory agency requirements
• Maintain professional licensure in good standing and complete all state-mandated continuing education requirements
• Follow all clinic guidelines outlining standards of personal appearance, attendance, and punctuality
