Job Overview/Summary

The Diabetes Care and Education Specialist will provide comprehensive diabetes self-management education, guidance, and support to individuals and groups. The specialist will work collaboratively with healthcare providers and patients to promote optimal diabetes management and improve patient outcomes through education and technology integration.

Responsibilities
  • Deliver diabetes self-management education and support to patients in various settings.
  • Assess individual learning needs and develop personalized education plans.
  • Collaborate with interdisciplinary teams to coordinate care and optimize treatment plans.
  • Provide training and guidance on diabetes technology, including glucose monitoring systems and insulin delivery devices.
  • Support continuous improvement in diabetes care practices through patient education and outcome monitoring.
Primary Skills
  • Strong knowledge of diabetes management and education principles.
  • Excellent communication and interpersonal skills for patient and team collaboration.
  • Proficiency with diabetes technology and digital health tools.
  • Ability to adapt education strategies to diverse patient populations.
Secondary Skills (Good To Have)
  • Experience working in community health or ambulatory care settings.
  • Familiarity with culturally sensitive education approaches.
Qualifications

Education:

Bachelor’s Degree in Nursing, Nutrition, or Pharmacy (Required).

Experience
  • Minimum 3 years of experience as a Registered Nurse, Dietitian, or Pharmacist (Required).
  • Minimum 3 years of experience in diabetes self-management education and support (Required).
  • General experience and working knowledge of diabetes technology (Required).
  • Experience working with a diverse patient population (Required).
  • 1 year of experience in community health or ambulatory care setting (Preferred).
Licenses/Certifications
  • PA Dietitian-Nutritionist, PA Pharmacist License, or PA Registered Nurse License (Required).
  • Certification as a Diabetes Care and Education Specialist (Required).
