We are seeking a passionate Occupational Therapist to join our team at One Health Home Health in Contra Costa Area. This is a full-time or part-time position in Alameda/Contra Costa, and we are looking for someone dedicated to providing top-notch care to our patients.

• Conduct patient evaluations and develop individualized treatment plans
• Provide occupational therapy treatments and interventions
• Document patient progress and communicate with healthcare team
• Educate patients and their families on treatment plans and techniques
• Collaborate with other healthcare professionals to ensure comprehensive care

• Valid Occupational Therapist license in the state of California
• Minimum of 1 year of experience in a healthcare setting
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Passion for providing high-quality care to patients
