We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team in providing exceptional home health care services to patients in Contra Costa County. One Health Home Health is committed to delivering high-quality care and improving the lives of our patients.

• Conduct patient assessments and develop individualized care plans
• Administer medications and treatments as prescribed by the physician
• Monitor and report changes in patient's condition
• Provide education and support to patients and their families
• Collaborate with the interdisciplinary team to ensure comprehensive care

• Current LVN license in the state of California
• Minimum of 1 year of experience in a clinical setting
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Reliable transportation and valid driver's license
