One Health Home Health in San Ramon, CA is seeking a dedicated After Hours Hospice nurse to join our team. This is an exciting opportunity to make a difference in the lives of our patients and their families.

• Provide compassionate and skilled nursing care to hospice patients during after hours.
• Assess patient needs and develop individualized care plans.
• Administer medications and treatments as prescribed by the physician.
• Communicate effectively with patients, families, and other healthcare team members.
• Document patient care and progress accurately and timely.
• As a member of the interdisciplinary team, the Registered Nurse provides skilled nursing services to patients served by Hinds Hospice. The registered professional nurse receives direction and supervision from the Clinical Manager and/or designee. This position will take calls on evenings, weekends or holidays from patients and families and make home visits as appropriate, attending to After Hours needs for our patients and families/Caregivers.
  
  Visits may include but are not limited to:
  Admission visits, Comprehensive visits, PRN visits, Imminent Death visits, Death visits, and Tuck‑in Visits.

• Valid RN license in the state of California.
• Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Compassionate and empathetic demeanor.
• Completed a professional nursing program accredited by the National League for Nursing (prefer completion of a baccalaureate program in nursing).
• One year experience in an acute care facility or community health agency; hospice experience a plus.
• Current RN/LVN licensure and CPR certification.
• Valid driver's license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment.
• Ability to meet the physical demands of the job (i.e., push, pull, lift/carry 35 lbs).
• Must be available:
  Thursdays/Fridays – 4 pm to 12 am and call time 12 am to 8 am;
  Saturdays – 8 pm to 8 am.
