Seeking a Nurse Practitioner to join a PACE (Program of All-Inclusive Care for the Elderly) interdisciplinary team serving an adult and elderly population (ages 55+).
Base pay range$/yr - $/yr
Location: Livermore, CA
Schedule: Full-time;
Monday-Friday 8a-5p
Duration: Permanent
Position Highlights- Provide adult Primary Care in a value-based model
- Seeing an average of 5–10 patients/day in a clinic setting
- Manage acute and chronic conditions, conduct assessments, and contribute to team-based care plans
- CA NP license (FNP, AGNP, or ANP), Furnishing License, DEA, CPR
- Open to new grads; experience in geriatrics or family practice a plus
- Must have at least 1 year of recent Primary Care experience
- Flexibility to cover other clinic locations as needed
- 403B retirement plan with 5% match
- Medical, Dental, Vision (through Sutter or Kaiser)
- PTO: 16 days (PTO/Sick) + 4 floating holidays + 10 recognized holidays
- After 2 years they get an additional 5 days (PTO/Sick)
- Annual bonus (either 6 or 9%)
