Full or Part Time Performer Dentist required 2–5 days per week for a STATE OF THE ART & INDEPENDTLY OWNED practice near Childwall, Liverpool

£15 per UDA + Private 50% Labs 50%
State of the Art modernised Surgeries
OPG, Intra Oral cameras, 3D Scanner
Fully computerised - 5 Surgery practice
Air conditioned
Well established patient list
Practice manager on-site
4/5 years experience required (ideally)
Available immediately

Full NHS Performer Number
Local Area Team confirmation
GDC certificate
UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
Indemnity certificate
Passport
Enhanced DBS (within the last 12-36 months)
Clinical references covering places of employment over past 2 years (minimum 2 referees)
Degree certificate
CPR/AED/Medical Emergencies certificate dated within the last 12 months
