Job Summary
We are seeking an experienced and driven Digital Transformation Lead to provide strategic and operational leadership for digital and service transformation across the PCN. This role is funded through the Additional Roles Reimbursement Scheme (ARRS) and will lead the development and implementation of digitally enabled, neighbourhood‑based care, supporting our practices to modernise services, improve access, and strengthen multidisciplinary team working.
Key Responsibilities- Lead digital transformation across the PCN, supporting practices to adopt modern, efficient, digitally enabled workflows.
- Support implementation of NHS England’s Modern General Practice Model and neighbourhood health framework.
- Strengthen multidisciplinary team working and support the development of Integrated Neighbourhood Teams.
- Improve access, operational efficiency and patient experience across all practices.
- Lead transformation and improvement projects across the PCN.
- Support practices to meet quality improvement requirements, including LQIS and other contractual or local priorities.
- Promote consistency, best practice and service improvement across all PCN practices.
- Work with system partners including community services, mental health, local authority and voluntary sector organisations.
- Support the safe implementation of digital systems and improved use of data to inform care and service delivery.
- Significant experience working in General Practice, Primary Care, PCNs or NHS transformation roles.
- Experience delivering digital transformation or service improvement programmes.
- Strong understanding of primary care operations and current NHS priorities.
- Experience implementing digital systems, operational change or service redesign.
- Excellent communication and stakeholder engagement skills.
- Ability to work across multiple organisations and professional groups.
- Strong organisational, leadership and problem‑solving skills.
- A proactive and collaborative approach.
- Experience with in a PCN, Integrated Care System or neighbourhood model would be highly advantageous.
