Position: Transitions Officer (2025/LM107)

Overview

Based across all College Campus sites, SR8 2RN
/ DH1 3SG
. Two Transitions Officers are required; one role based at Houghall Campus and the other at Willerby Grove. The Transitions Officer supports the successful transition of learners with Special Educational Needs and Disabilities (SEND) and/or high needs from school into college and from college into employment or other progression pathways.

This role strengthens transition planning and communication with all stakeholders in line with SEND reforms, SEND code of practice and changes to the Ofsted toolkit, ensuring applicants and learners experience a timely, smooth, well-informed and supportive transition into college life prior to enrolment.

The post holder acts as the first point of contact for schools and a key link between external partners and internal college teams, ensuring accurate information about learner needs is shared in a timely way so that appropriate support, planning and provision can be put in place.

Responsibilities

Qualifications
  • GCSE level standard education or equivalent (maths & English).
  • Level 3 qualification or equivalent experience in education, SEND, health and social care, youth work, careers guidance, community support, or a related field.
