Band 5 Dietitian: 1 WTE for permanent position (37.5 hours a week, 8am-4pm)
An exciting opportunity has arisen for a permanent Band 5 Dietitian within the nutrition and dietetic department at the Walton centre.
The Role will involve contribution to the delivery of the nutrition and dietetic service across The Walton Centre NHS Foundation Trust. You will be involved in the assessment and treatment of a defined caseload of patients and maintain accurate records. You will contribute to the service development at the Walton centre.
Main duties of the job1. To undertake nutritional assessment and calculate nutritional requirements of referred in-patients with neurological conditions at the Walton Centre, taking into account medical, social and cultural factors
2. To assist in the provision of the dietetic service to specialist clinical areas under supervision of the specialist dietitian as part of training and CPD
3. To provide nutrition and dietary advice suitable for the patients medical, social and cultural requirements which is evidence-based
4. To negotiate change with patients to enable them to achieve dietary targets set
5. To monitor and review nutritional care plans against outcome measures and adjust care plan to facilitate achievement of goals
6. To assist in developing a range of educational resources which have a clear evidence-base and which are acceptable to users of the service and their carers
7. To accurately record all assessments and interventions in medical notes, nursing documentation and dietetic records in line with Trust policy. To communicate in writing with other agencies, e.g. GPs, community dietitians etc., about patient care
8. To ensure safe and timely discharge of patients on home enteral feeding liaising with appropriate internal and external agencies
9. To advise pharmacy and senior dietitians regarding stock levels of nutritional products at ward level
10. To communicate and liaise with other agencies appropriately, e.g. members of the MDT, catering, pharmacy, community dietitians.
