Job Description3 months contract with local authority
The role is part of Islington Provider Services Day Opportunities and Hubs, focusing on empowering individuals to live independently by leveraging their strengths. The services cater to adults over 18 with diverse needs, including learning disabilities, physical and sensory needs, mental health issues, and older adults. The service is undergoing transformation to become more multi‑agency, with potential changes aligned with service development.
The position involves providing out-of-hours social activities and fostering friendships for individuals, carers, and families. Flexibility across Provider Services is required to meet service needs. The role supports individuals in accordance with personalised plans, emphasizing independence and collaboration with health and social care professionals.
- Deliver care and support aligned with personalised support plans.
- Perform personal care, domestic tasks, and facilitate independent living skills.
- Work in an enabling manner to maximise independence.
- Collaborate closely with health and social care professionals involved in care planning.
- Report directly to the Team Leader or Assistant Manager.
- Exemplify best practices in strengths‑based and person‑centered approaches.
- Connect individuals with informal, community‑based support.
- Engage in partnership with residents, carers, community organisations, and professionals.
- Participate in occasional weekend or evening events on a voluntary basis.
- Experience working with adults with diverse needs, including learning disabilities, physical and sensory needs, mental health issues, and older adults.
- Ability to work in a strengths‑based and person‑centered manner.
- Strong collaboration skills with health and social care professionals.
- Flexibility to work across different services and adapt to changing service needs.
