Position: PCN Clinical Pharmacist (ARRS)

Job Summary

Central and South Knowsley PCN is recruiting Clinical Pharmacists as part of our planned transition to a directly employed PCN pharmacy workforce. These roles are funded through the Additional Roles Reimbursement Scheme (ARRS) and employed by Roby Medical Centre, the PCN employing practice. Post holders will work across up to two PCN practices, supporting medicines optimisation, structured medication reviews, long‑term condition management and medicines safety.

The role sits within a robust PCN governance and supervision structure
, including a named Lead Clinical Supervisor and Senior Clinical Pharmacist, ensuring clear clinical oversight, professional support and development. Permanent, subject to continued ARRS funding. Full time or part time considered.

Across up to two GP practices within the PCN.

Main duties of the job

Key responsibilities

  • Undertake Structured Medication Reviews (SMRs), particularly for patients with polypharmacy, frailty, long‑term conditions and high‑risk medicines.
  • Provide expert medicines optimisation advice to GPs and the wider multidisciplinary team.
  • Prescribe and deprescribe within scope of practice, in line with NICE guidance, local formularies and PCN priorities.
  • Support long‑term condition management, including cardiovascular disease, diabetes, respiratory disease and frailty.
  • Monitor and manage high‑risk medicines, including anticoagulants, DMARDs and controlled drugs.
  • Contribute to antimicrobial stewardship and prescribing quality improvement.
  • Participate in audits, significant event learning and PCN medicines safety activity.

Work collaboratively across practices to ensure consistent, high‑quality pharmaceutical care.

Job description

As a Clinical Pharmacist within the PCN, you will work as part of an integrated healthcare team, supporting GPs, nurses and other healthcare professionals to deliver high‑quality, safe and effective prescribing. You will be responsible for medicines management, structured medication reviews (SMRs) and improving patient outcomes through proactive, evidence‑based pharmaceutical care. You will also be a non‑medical prescriber and contribute to chronic disease management, supporting patients with polypharmacy and deprescribing where appropriate.

Specification Qualifications (Essential)
  • Registered Pharmacist with the General Pharmaceutical Council (GPhC).
  • Independent Prescriber qualification (or completion by start date).
  • Successful completion of the CPPE Primary Care Pharmacy Education Pathway (PCPEP) – or at least 80% completed at the point of application.
  • Applicants must clearly evidence completion status. Applications that do not meet this requirement will not be shortlisted.
Experience (Essential)
  • Experience working in primary care or general practice.
  • Strong understanding of NICE guidance, medicines optimisation and prescribing governance.
  • Ability to work flexibly across more than one PCN practice.
  • Confident use of GP clinical systems (EMIS).
Desirable
  • Experience leading or contributing to medicines optimisation or quality improvement projects.
  • Experience supporting or supervising pharmacy technicians or trainees.
  • Knowledge of PCN and ARRS delivery models.
