Position: Team Manager – Connect to Community (West) - LD&A and MH

Overview

Starting salary: £61,954 per annum, 36 hours per week (pro rata for part time).

We are hiring a Team Manager to join the Connect to Community (C2C) management team (West) within the Adults, Wellbeing & Health Partnership Directorate. You will provide leadership to mental health and learning disabilities social care professionals and help shape a modern, transformative service focused on early intervention and prevention.

About Our Service

From March 2026, our brand‑new Connect to Community operating model will reshape how residents access Adult Social Care by offering timely, strengths‑based and person‑centred support. C2C will bring adult social care professionals into a dynamic multidisciplinary team, leading urgent assessments, safeguarding, risk management and short‑term interventions, with access to reablement, housing advice and technology‑enabled care. The model aims to reduce delays, avoid unnecessary handoffs and enable early action with consistent, high‑quality support.

C2C works with Area Teams to create a seamless pathway for people with complex needs and collaborates with specialist Learning Disabilities and Autism teams, Mental Health teams and Acute Hospital Teams. Team Managers may be required to work flexibly across services as needed.

About

The Role

You will lead a proactive group of mental health, learning disability and autism practitioners, using a strengths‑based approach to assessments and support planning. You will work with adults with diverse and complex needs to help them live meaningful lives within the community. You will ensure that Connect to Community delivers a high‑quality, customer‑focused service while taking responsibility for leading the team and managing resources.

Your remit includes risk management, business continuity, forging relationships with other agencies and contributing to service improvement initiatives.

A social work, occupational therapy, nursing or equivalent health qualification with specialist knowledge/experience in mental health and learning disabilities is essential. You will bring experience in successful staff management, including performance management, development, and addressing conduct, attendance and capability issues. You will supervise Assistant Team Managers and Senior Practice Leads, supporting professional growth, performance, recruitment, succession and staff retention. You should have budget management experience and a solid understanding of personalisation.

  • A professional qualification in Social Work, Occupational Therapy, Nursing, or another relevant health/social care field (including registration where required)
  • Extensive experience in adult social care with knowledge of mental health and learning disabilities; strong understanding of statutory responsibilities and relevant legislation
  • Experience in managing, motivating, coaching and developing staff
  • Ability to manage performance, support reflective practice, and lead high‑quality service delivery
  • Experience building effective partnerships with diverse stakeholders
  • Ability to handle complex casework, risk, safeguarding and operational challenges
  • Strong organisational skills to monitor performance, manage resources and support improvement
  • Commitment to the personalisation agenda and strengths‑based practice
