Care Navigator / Medical Receptionist

We are looking for a Care Navigator/Medical Receptionist for our Primary Care Hub and GP practices.

Location & Contract

Primary Care, Garston Family Health Centre

Contract:

Permanent
Salary: £18,067.55 per annum

Shifts:

Monday to Friday 08:00–18:30

Hours:

27.5 per week

Responsibilities
  • Provide care navigation call handling duties on behalf of the PC24 practices, following PC24 processes and procedures.
  • Open and close premises, complete security checks and ensure all clinical rooms are prepared.
  • Direct calls to the most appropriate clinician and service; identify and escape emergencies to ambulance services.
  • Greet patients and visitors professionally and log attendance via the system.
  • Answer questions and queries that arise.
  • Provide administrative support to the wider primary care team and follow up calls to other services as required.
  • Support the onsite practice and clinical teams.
  • Record essential information accurately using EMIS Web system.
  • Liaise with other services to request instructions from the clinical team and log all information.
  • Understand local referral arrangements/pathways for holistic support from a wide range of sectors.
  • Maintain patient confidentiality at all times.
  • Adhere to PC24 Standard Operating Procedures.
  • Display patience, empathy and compassion via telephone and face‑to‑face interactions.
Qualifications & Experience
  • Excellent verbal and written communication skills.
  • Ability to work as part of a team and independently.
  • Understanding the importance of accurately recording confidential data.
  • Good IT skills, including Microsoft Office and EMIS.
  • Previous experience in call handling and reception positions.
