dis Ability Cornwall & Isles of Scilly is seeking a Personalised Support Specialist to help adults in North Cornwall live independently by providing tailored support and practical assistance. You will conduct home visits, perform holistic assessments, and coordinate housing, welfare benefits, and local services to improve wellbeing.
The role focuses on holistic support, client independence, and safety, with 37 hours per week, Monday to Thursday 9am–5pm and Friday 9am–4:30pm.
