The Walton Centre NHS Foundation Trust is seeking a Rehabilitation Co-ordinator to act as the Single Point of Contact for referrals within the Cheshire and Merseyside Rehabilitation Network. You will provide expert clinical advice, coordinate access to rehabilitation services, and support transfer planning across acute, community and social care settings.
Working with multidisciplinary teams, you will review referrals, undertake assessments, monitor patient pathways and help reduce delays.
