The contractual hours are 25 hours per week. For contracts of fewer than 35 hours per week, salaries and any allowances will be paid on a pro rata basis. If you join us in this role, you will start at Grade 4. Don’t worry we will fully support you to develop your skills and progress. As part of the role, you will complete a structured training programme that includes:

  • Strength‑Based Approaches
  • Liquid Logic training
  • Care Act e‑Learning
  • Assistive Technology / Technology‑Enabled Care
  • Strength‑Based Approaches (Practitioner level)
  • Goal Setting and Risk Assessment
  • Financial Assessment Training

Once you’ve successfully completed all of the required training and are confident in the Grade 5 responsibilities, you will move up to Grade
5. Most people complete this progression within 12 months, and your manager will work with you to make sure you have everything you need to get there.

The role

The Independence and Reablement Practitioner will play a key role in helping people to achieve independence, relearn skills and build confidence, empowering people to live physically, emotionally, and socially full lives reaching their aspirations. As an Independence and reablement practitioner you will use a strengths‑based approach, to work directly with individuals over a short and focused period to develop a person‑centred goal plan and ensure holistic support is given.

You will work with people over the age of 18, who may have had a period of ill health or difficulties to maintain or regain the skills needed to enjoy their every‑day life. The Independence and Reablement Practitioner will work alongside a highly skilled team of professionals, including Occupational and Physio Therapists, Social workers, Social Care Assessors, Senior Independence & Reablement Practitioner and Team leaders.
