Citizens Advice Liverpool is recruiting Link Workers to support GP teams and patients through social prescribing and coordinated referrals across health and community sectors. The role involves meeting patients in health centres, community settings, and homes to develop tailored plans that improve health and wellbeing.
You will work with a wide network of agencies to connect patients to local resources and provide ongoing signposting and support as part of the Citizens Advice Liverpool service.
