An exciting opportunity has arisen for a part time fixed term Team Manager to join Adult Social Care & Health in our Adult Access Team.
Adult Access is the front door of Adult Social Care & Health which enables a clear direction for enquires resource, demand management, and accountability.
Adult Access has four areas of operations to enable the delivery of the wider Adult Social Care Target Operating Model. These functions are:
- Multi-Disciplinary Team (MDT):
Adult Social Care and Occupational Therapy. - Independence and Reablement.
- Safeguarding Triage.
All team functions will focus on a seamless person-centred, strength-based approach from first contact. In practice, Wellbeing Advisors, Social Workers, Social Care Assessors and Occupational Therapy will work together as a team. The aim being to prevent, reduce and delay people needing formal and longer-term care, and to provide short term reablement support, where required, to enable people’s independence.
The Role
- As a Team Manager within a fast-paced Multi-Disciplinary Team (MDT), your role is to provide leadership, direction, and support to ensure the team delivers high-quality outcomes for customers and stakeholders.
- You, as a Team Manager will lead and motivate a team of professionals from different disciplines. You will support decision-making and resolving complex cases or issues.
- Success as a Team Manager requires strong leadership, excellent communication skills, sound judgement, organisational ability, and the capacity to balance competing priorities while supporting both team performance and customer outcomes.
- Through every interaction as Team Manager, apply the Prevent, Reduce, Delay principles of the Care Act 2014 to promote independence and wellbeing.
This role is fixed term to meet a temporary business need, ensuring management capacity and service delivery during a defined period while maintaining flexibility for future workforce planning. The position is made up of 2 part-time Team Managers. Each working 17.5 hours each week and will end 12 months from start date.
