Job Summary
Fern Ward is a 16‑bed complex care dementia unit in Leigh Moss Hospital, Liverpool. The unit is part of Mersey Care NHS Foundation Trust, which serves over 1.4 million people across the North West, North Wales and the Midlands. The ward provides high‑standard assessment and treatment for people with dementia and BPSD (Behavioural and Psychological Symptoms of Dementia) and offers a multidisciplinary team of occupational therapists, psychiatrists, physiotherapists, activity workers and assistant practitioners.
Mainduties
The Nursing Associate works with the multidisciplinary team and the Registered Nurse to assess, plan, organise and deliver clinical care in line with patient needs, as agreed with supervising clinicians.
Key responsibilities- Deliver high‑quality, person‑centred care under the direction of a Registered Nurse or other registered care professional, dependent on setting.
- Have proficient attitudes and behaviours compatible with NHS Values.
- Work as part of a designated clinical and care team delivering care that focuses directly on the needs of the person.
- Carry out delegated clinical and care tasks to a high standard and competency, under the direction and supervision of a Registered Nurse or other registered care professional.
- Where appropriate work without direct supervision, at times delivering care independently in line with the individual’s defined plan of care, accessing clinical and care advice when needed.
- Continue to develop and improve practical and theoretical knowledge, competence and skills whilst in clinical practice and maintain all evidence required.
- Contribute towards developing a culture of learning and innovation, developing high‑quality learning environments.
- Holistically support people to improve and maintain their mental, physical, behavioural health and wellbeing, promoting health and preventing ill health.
- Assist the registered nurse in the ongoing assessment, planning, management and evaluation of care.
- Recognise changes in a patient’s condition that require the intervention of others and refer to the supervising registered nurse and senior colleagues for reassessment.
- Work in collaboration with others to ensure the safe and timely discharge of patients, avoiding readmission.
- Perform procedures as detailed within the NMC Standards for proficiency for Nursing Associate (2018).
- Administer medications and undertake annual medicines management training.
- Act as an advocate for the patient and the family, promoting and protecting their interests, privacy and rights, equality and inclusion.
- Participate in Trust initiatives to improve patient safety and care.
- Adhere to Trust policies relating to medication control and administration.
- Maintain high standards of nursing care and act when standards are not being maintained.
- Work within the NMC code of professional practice and Trust procedures, policies and guidelines.
- Be flexible to work within areas or departments to meet changing service needs.
- Communicate with the team leader/nurse in charge and collect data to support smooth department operations.
- Support junior and senior colleagues, resiliently managing change.
- Assist in risk assessment and minimisation, ensuring incidents and near‑misses are reported appropriately.
- Manage verbal complaints, refer when unable to resolve, and participate in incident/complaint investigations.
- Establish and maintain effective verbal and written communication in all contexts.
- Recognise, prevent and manage conflict where possible, reporting to the team leader when unable to resolve.
- Participate in staff meetings regularly and provide support and supervision to health care support workers.
- Receive delegated duties from the supervising Registered Nurse and senior colleagues, adhering to NMC delegation and accountability documents.
- Foundation Degree in Nursing Associate practice
- Current NMC Registration
- Previous experience working within a healthcare setting.
- Experience working in teams under appropriate supervision as part of a multidisciplinary team.
