Healthcare Nursing, Clinical Nurse Specialist
We are looking for a Band 6 District Nursing Caseload Holder. Working across Liverpool, Sefton and Knowsley areas, who would relish the challenge of working within primary care and caring for patients with complex needs whilst working towards an integrated healthcare structure. We are looking for candidate's who are self-motivated, keen to work in a progressive division and to develop their skills and knowledge in supporting patients who are housebound with long term conditions and complex needs.
The service operates 365 days per year to cover our 8am-8pm 7 day a week model.
Main duties of the jobThe post holder will have continuing responsibility for the management of a defined caseload. They will be responsible for the continuity of care and be the lead person for communication to identified stakeholders. The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload. This will be done via a holistic assessment of care needs and the provision of clinical care for patients/clients with acute and chronic highly complex needs.

Job responsibilitiesThe successful candidate should be able to work autonomously while demonstrating a team approach, and have strong clinical leadership firmly embedded within their role. We will also require excellent communication skills, clinical decision making skills and a problem solving approach. The successful candidate will be well supported, developed, and have the opportunity to work in an organisation that demonstrated inclusive and supportive leadership.
As per NMC guidance for Standards for student supervision and assessment, applications will be considered for those without mentorship qualification; the candidate must be able to demonstrate the following within their application:
- Interpersonal communication skills, relevant to student learning and assessment
- Conducting objective, evidence based assessments of students
- Providing constructive feedback to facilitate professional development in others, and
- Knowledge of the assessment process and their role within it.
- Have an understanding of the proficiencies and programme outcomes that he student they assess is aiming to achieve.

Person Specification Qualifications- Registered Nurse
- Teaching/mentorship qualification or equivalent or equivalent work based experience.
- V300 Prescriber, if required for the post
- Demonstrable post registration/qualification experience.
- Experience of successful multiagency working
