An exciting opportunity to join our Integrated Heart Failure Service at NHS University Hospitals of Liverpool Group to support the heart failure service on the Royal Liverpool Site on a 37.5 hour basis over 7 days. We are seeking a highly motivated individual with excellent interpersonal and communication skills. The successful candidate will support band 7 clinical nurse specialists within the current team.
The role involves close collaboration with the heart failure multidisciplinary team, providing patient education, supporting symptom management and reducing unnecessary hospital admission, while maximizing quality of life. Flexibility and awareness of heart failure are essential in a proactive, dynamic, and committed team environment.
Main duties of the job- Review and assess patients with heart failure
- Administer intravenous medication
- Care and maintenance of vascular access devices
- Perform peripheral cannulation
- Check daily weight and assess improvements in leg swelling and symptoms
- Signpost or refer to other community or acute services
- Order, take, and review blood samples and investigations
- Communicate within the MDT and with external organisations
- Manage and organise referrals
- Record data into the service database

Jobresponsibilities
Perform specialist assessment of patients’ care needs, plan, implement and evaluate care delivery using clinical knowledge and skills. Provide emotional and psychological support to patients and carers. Perform point‑of‑care blood tests, review and interpret results and raise concerns. Be a specialist in peripheral venous cannulation. Identify opportunities to promote health education and informed choice for patients and their families. Maintain accurate and legible patient records in line with Trust policy and Nursing and Midwifery Council guidelines.
Recognise changes in patient condition that may require intervention by others and refer appropriately. Support and encourage the multidisciplinary team in resolving patient problems through innovative case‑management models. Deliver training to patients, carers and staff in hospital settings regarding IV therapy and IV access. Work within the NMCs Scope of Professional Conduct.
