An opportunity has arisen for a part‑time Band 6 Glaucoma Nurse Specialist within St. Paul’s Ophthalmic Outpatients.

Responsibilities- Perform specialist assessment of patients’ nursing needs, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills.
- Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation.
- Co‑ordinate and manage nurse/AHP‑led clinics.
- Contribute to a multi‑professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.
- Establish and maintain effective communication with patients, carers and relatives.
- Support patients and their families, ensuring they receive the required information to enable self‑management, participation in care delivery.
- Recognise changes in patients’ condition that require intervention of others and refer on as appropriate.
- Support, assist and encourage the multi‑disciplinary team in resolving patient problems by providing new and innovative models of case management.
- Lead the specialist nursing contribution to the service, working in partnership with lead clinicians, General Managers and the Matron to ensure delivery of high standards of patient care.
- Work with the Matron to promote the development of nurses/AHPs undertaking the CNO ‘Ten Key Roles’ as appropriate (e.g., prescribing).
- Deputise for the Band 7 manager as required.
- At least 2 years experience working within an ophthalmic setting.
- A relevant Ophthalmic Nurse Specialist qualification.
- Experience in nurse‑led glaucoma outpatient clinics preferred.
- An ANEX 21 training post may be considered to gain required competencies; 75 % of the Band 6 salary will be paid until completion of competencies.
