successful candidate will be part of the frailty assessment unit

consisting of a ambulatory unit providing direct conveyance from community and accident and emergency with a clinical frailty score between 4 and 7. This is a 7 day service from 07:30 to 20:00.

this is an multi disciplinary approach to assessing and promoting frailty service within the organisation

the successful candidate will also be expected to rotate to a 12 bedded in patient frailty assessment unit that consist of a 72 hours stay unit that supports our assessment area and provides comprehensive geriatric care to over our 65 year old patients.
