Adult Nurse Case Manager RGN - Complex Care Case Manager | Liverpool | Up to £40,000 DOE  Quarterly Bonus | Monday-Friday Ready to get off the rota without leaving complex care behind? If you're an RGN with complex care experience - including tracheostomy and ventilator care - this could be a great next move. We're looking for an Adult Nurse Case Manager to join a leading national complex care provider, based from their Fazakerley branch and supporting packages across Merseyside and West Lancashire.

You'll take ownership of a caseload, combining your clinical expertise with leadership, assessment and care management. What you'll be doing:
Managing and reviewing complex care packages Completing clinical assessments and care plans Visiting clients in their own homes Providing clinical leadership to care teams Supporting and mentoring nurses and support workers Working with families and wider healthcare professionals Ensuring high standards of clinical governance and care

What you'll need:
Registered General Nurse with active NMC registration Adult complex care experience Recent tracheostomy and ventilator care experience Experience supervising or supporting care staff Strong clinical assessment skills Full UK driving licence
