Community Nurse

Procare Community Services seeks a registered nurse for its adult community nursing team, working with nurses and health care assistants under a District Nurse. The post holder visits adults at home, assessing and treating a caseload with clinical tasks including wound care, catheterisation and end of life care. Work runs seven days a week, 8am to 8pm, with a review for a Band 6 senior post once competencies are met.

• Visit adult patients in their own homes who have a nursing need, including long-term, complex and end of life care
• Assess, implement and evaluate person-centred care for a caseload of patients
• Carry out venepuncture
• Provide wound care
• Carry out catheterisation
• Manage syringe drivers
• Provide end of life care and symptom control
• Prioritise visits according to patient clinical need
• Organise own workload
• Record care on the EMIS electronic patient record
• Work as part of a local team with the acute hospital, social care, mental health teams, GPs and practice nurses
• Work under the direction of a District Nurse

• Registered Level 1 Nurse

• Current Nursing and Midwifery Council (NMC) registration

• Ability to assess nursing needs and implement nursing care and treatment

• Ability to prioritise visits in line with patient clinical need
• Ability to organise own workload without direct supervision

• Full UK driving licence and access to a vehicle for business use, where required for the post
• Independent right to work in the UK, as the employer does not sponsor visas
