About the Role

We are recruiting for 1 WTE (37.5hrs) and 0.8 WTE (30.0hrs) Family Nurse positions located at Yew Tree Centre, L14 4ED
. Successful candidates will attend external residential FNP training (3rd‑5th November 2026) to support learning and competencies. The role is part of Liverpool Family Nurse Partnership and involves delivering the FNP intensive, preventive home‑visiting programme to vulnerable young women and parents aged 19 years or under, plus care leavers up to 24 years old.

Key Responsibilities
  • Recruit and engage eligible and hard‑to‑reach pregnant young women into the FNP programme.
  • Use programme materials and methods in clients’ homes to improve pregnancy outcomes, children’s health and development, and parental life‑course.
  • Work therapeutically with family members to promote behaviour change and positive outcomes for children and families.
  • Take a lead professional role when required for children and young people with additional needs who require an integrated package of support from more than one practitioner/service.
  • Monitor and reflect in supervision to maintain clinical expertise and ensure the service is of high quality and implemented with fidelity.
  • Collaborate with local statutory and mainstream services so families can access a range of universal health and support services and graduate from the programme when the child turns two years old.
  • Provide leadership expertise within your locality and the wider service, contributing to quality assurance and improvements.
Working Hours & Travel

The service operates within core hours 8:00‑17:30, Monday to Friday, with flexibility to meet client needs. Travel across Liverpool and, on occasion, to neighbouring boroughs is expected.

Working Arrangements

This position offers hybrid working opportunities; a requirement to visit the base frequently for peer support, material preparation, weekly team meetings and supervision.
