Carers Support Worker (Carers Assessments)
POSITION:
Carers Development Worker - Mat leave cover (01.09.26 to 28.05.27)
LOCATION:
Liverpool
HOURS:
40 hours per week
As a member of Liverpool Carers Centre team, you will work with unpaid carers to support them to access practical and emotional support. As a Carers Development Worker, you will aid in empowering carers to take control of their own health and wellbeing. You will give time and focus on the what matters to me philosophy and take a holistic approach to connect carers to community groups and statutory services.
Coreresponsibilities of the role
- To support carers through the Carers Assessment process, primarily via home visits or other appropriate contacts.
- Complete Carers Support Plans with carers using a person‑centred approach.
- Provide carers with information on the range of services available and how to access them.
- Complete and submit carers support plans in a timely fashion to ensure departmental KPIs are achieved.
- Liaise with colleagues in statutory services.
- Be an energetic and solution‑focused worker, supporting carers to access the most appropriate services for their caring role.
- Develop relationships across the city that help and support unpaid carers, acting as a main enabler for holistic solutions.
- Developing and sustaining links with partnership organisations to encourage collaborative working to support unpaid carers.
- Providing advice, guidance, and practical support in the implementation of support packages.
- Collating, analysing, and reporting on opportunities for organisational learning on the Carers project.
- Work collaboratively with key stakeholders to raise awareness of carers known to them.
- Promote the referral pathway for carers.
- Support carers to access the relevant support based on their need.
- Take part in presentations, attend meetings, and provide information at events and activities offering support to carers.
- Provide monthly KPIs and reports as required.
- Adhere to data protection legislation and data sharing agreements, ensuring accurate recording of serviced carers.
- Meet internal or external demands for information to demonstrate compliance with targets, standards, statutory, regulatory, or other requirements.
- Maintain Local Solutions profile by providing excellent customer‑focused support to all internal and external parties.
- Carry out any other duties appropriate to the post, as necessary or as requested.
- Participate in professional development, using appropriate learning opportunities, to improve relevant knowledge and skills.
- Ensure all activities of the project provide equality of opportunity.
- Ensure policies and procedures are implemented within working practice.
- Experience working in health or social care.
- Experience supporting unpaid carers or working with people.
- Experience working as part of a team, prioritising own workload, managing diary, building relationships with carers and professionals, and managing databases and inputting information.
- Availability to attend occasional evening or weekend events.
- Technical skills:
Microsoft Office, Outlook. - Successful candidate will require an enhanced Disclosure & Barring Check and undertake mandatory safeguarding training.
- Travel is required; a car owner/driver is essential for this city‑wide post.
