Job Description - Physical Therapist - PRN )
Livingston Regional Hospital
ResponsibilitiesUnder indirect supervision, provides direct patient care to individuals with physical disabilities and/or functional limitations in the outpatient and inpatient settings. Plans therapy programs, directs and assists patients in exercises and use of wheelchairs and other devices, and evaluates patient progress. Educates patients and families in appropriate physical therapy methods. Leads the work of and serves as a resource for other therapists.
Must be willing to flex schedule to work other shifts as needed and to work other than usual areas as needed, if qualified for duties that are required for that position.
Qualifications- Observes professional ethics in maintaining confidential information concerning the financial, personal, medical or employment status of patients/families at LRH.
- Must follow and enforce accepted safety practices for patients and hospital.
- Must report safety hazards and initiate appropriate action. Must participate in safety instructional programs.
- Provides patient care based upon the patient’s needs and medical assessment, written evaluation and plan of care condition.
- Provides treatment and monitors the patient’s response to rehabilitation in compliance with the hospital and physical therapy department policies and procedures.
- Supports the physical therapy department and hospital, and at all times strive to be an integral and effective member of the team, including active participation in the Physical Therapy dept PI plan and the hospital’s commitment to continuous quality improvement. Assist with data collection, makes suggestions for improvement, documents correctly
- Completes assessment, plan, goals and documents within the set time frames for the care setting.
- Evaluates patient’s response and progress to prescribed therapy treatments.
- Revises and updates plan of care and goals in a timely manner.
- Identifies and documents further rehabilitation potential if treatment is to continue.
- Performs the discharge summary in a timely manner.
- Encourages participation in and educates the patient and family on treatments, purpose and goals.
- Communicates with the physician and other healthcare team members on the patient’s progress and outcome from the program.
- Performs only physical therapy functions permitted by the practice act and does not exceed those responsibilities.
- Demonstrate ability to handle problem areas and consults with peers as needed.
- Delegates appropriate responsibilities to Physical Therapy Technicians and PT Assistants with reviews and monitoring for completeness and accuracy.
- Assists in clinical supervision and teaching of students, volunteers and observers.
- Complies and adheres to established physical therapy guidelines, protocols and duties.
- Establishes priorities based on available space, time, staffing and resources.
- Performs safety, environmental and/or infection control methods.
