Hospice RN Case Manager
Join Trinity Health At Home as a Hospice RN Case Manager and deliver compassionate, technology‑driven care to patients with life‑limiting or terminal illnesses in their homes. You will conduct patient assessments, manage symptom control, and oversee overall patient care and outcomes.
Schedule and ShiftMonday‑Friday, 8:00 a.m. – 5:00 p.m., with weekend/holiday rotation as required.
Key Responsibilities- Provide one‑to‑one home care for patients.
- Focus on a patient‑centered approach to pain and symptom management.
- Collaborate with supportive, motivated colleagues in an inspiring environment.
- Maintain flexibility in scheduling and patient workload.
- Leverage advanced, easy‑to‑use technology to enhance care delivery.
- Graduate of an approved nursing education program.
- Licensed Registered Nurse in the State of Connecticut.
- One (1) year of experience as a professional acute‑care nurse.
- Hospice or home‑care experience preferred but not required.
- $10,000 sign‑on bonus and Day 1 benefits.
