Job Title: Medical Coordinator

Starting Hourly Pay Rate: $17.15
Reports to: Home Administrator

The Medical Coordinator is primarily responsible for the health, medical care and well-being of each person in the home/s that he/she is assigned.
Job Specific Responsibilities- Assures medical, dental and other needs are met. Manages and documents the administering of medication and handles medical appointments and necessary follow-up documentation.
- Guides and assists with personal and medical care needs.
- To the extent possible, schedules appointments to assure the least amount of conflict with other scheduled activities. Posts appointments on a calendar that can be reviewed by staff.
- Provides, or when appropriate, offers individuals supported by COC with instruction in the skill areas of:
- Self-sufficiency
- Nutrition, meal preparation
- Serving meals & clean-up
- Physical health
- Social Development
- Employment
- Community Resources
- Emotional Maturity
- Communication
- Leisure time activities
- Interdependence
- Transportation

Position Requirements Qualifications- Must be at least 18 years old.
- High school diploma or equivalent.
- Have a valid Michigan driver's license and maintain the ability to be insured by COC's insurance carrier.
- Proficient with written and verbal communication skills.
- Complete and maintain all required training.
- Be in good physical health with the capability of transferring and/or lifting persons who require physical assistance.
