Welcoming hospice company seeking a full time RN Case Manager M-F Days. Company car, supportive team, generous benefits!

• Provide nursing care to patients in hospice care in patient homes and facilities
• Develop and implement individualized care plans for patients and their families
• Coordinate with interdisciplinary team members to provide comprehensive care to patients
• Administer medications and treatments as prescribed
• Monitor and assess patient symptoms and provide comfort measures as needed
• Educate patients and their families on end-of-life care and support
• Document patient care and progress in electronic medical records
• Provide emotional and spiritual support to patients and their families
• Advocate for patients and their families to ensure that their needs are met
• Participate in on-call rotation as needed

• Active RN license in the state of practice
