Licensed Practical Nurse; LPN HOSPICE

Job Summary

Licensed Practical Nurses work closely with supervising physicians/registered nurses to assess and monitor patients’ needs. They ensure that treatment and care are conducted according to the procedures of the facility, as well as administer medications and perform other related tasks.

• Assess clients’ condition during every visit and trackany observations
• Take and record measurements of vital signs such asblood pressure, temperature, heart rate etc.
• Administer medication as prescribed by the client’sphysician
• Dress or redress open wounds and assess progress of healing
• Continuously educate clients, their families andother caretakers on proper home health care strategies and procedures
• Provide palliative care as needed to keep the client comfortable
• Make recommendations for devices or tools thatmight improve the client’s quality of life
• Listen to clients and be prepared to respond to concerns or requests

• Current nursing license in the state
• Must know Kinnser and Axxess
• Current CPR (American Heart Association)
  Certification
• Access to reliable transportation
• Ability to work independently and with directions
• Compassionate and friendly demeanor
• Experience with Microsoft Office, Excel, Outlook andWord to document and track client care activity
