, a premier long-term healthcare facility, and make a meaningful difference in the lives of our patients. As a Licensed Practical Nurse (LPN), you will have the opportunity to work with a compassionate and dedicated team to provide high-quality care to our residents.
We are seeking a dedicated and compassionate LPN to join our team on a full-time basis, working day shift (7am-7pm). As an LPN, you will be responsible for providing direct patient care, conducting assessments, and communicating effectively with patients, families, and healthcare teams.
Responsibilities and Duties:- Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
- Perform admission duties, including completing assessments, MAR, TAR, POS, reviewing chart set-up, and ordering medications
- Manage discharges, including completing documentation, notifying physicians and guardians, and arranging transportation
- Provide wound care, following protocols, irrigating, packing, and removing dressings
- Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
- Respond effectively to emergency situations
- Licensure/Certification: LPN
