What you will do:

  • Conduct comprehensive assessments to determine the care needs of individuals and families within North Powys.
  • Develop and implement personalised care plans that promote independence and wellbeing.
  • Work collaboratively with multidisciplinary teams, including healthcare professionals, to ensure holistic care provision.
  • Provide guidance and emotional support to individuals coping with various life challenges.
  • Maintain accurate records and reports in line with regulatory and legislative requirements.
  • Regularly review care plans to ensure they continue to meet the needs of service users.
