This is an exciting opportunity to work with a well developed multidisciplinary Team in the care of patients with Rheumatological conditions.

The post holder will be responsible for the assessment, development, implementation and evaluation of programmes of care for rheumatology patients.

The post holder will be required to liaise with nursing and medical colleagues and other members of the Multidisciplinary Health Care team to ensure the delivery of effective patient care. The post holder will be expected to work within the hospital and community setting as dictated by service needs.

The ability to speak Welsh is desirable for this post;
English and/or Welsh speakers are equally welcome to apply.

• Essential: NMC registered nurse
• Desirable:
  Other qualification specifically to specialty

• Essential criteria:
  Substantial experience at band 5
• Desirable criteria:
  Experience of working within an MDT

• Essential Criteria:
  Understanding of Quality/Clinical Governance.

• Desirable criteria:
  Ability to speak Welsh
