Sunderland East Primary Care Network are excited to advertise a new role in our network.
The post is for 37 hours, however will consider 2x part time staff members for the right candidate.
The successful candidate will need to be organised,self-motivated, flexible, and comfortable with adapting their workload torespond to changing and conflicting priorities. You will be expected to support the 7 member practices at any given time and therefore building effective relationships is key to this role as well as being adaptable to change.
The role will be based around all 7-member practice, allbased within Sunderland East area. (All member practices listed below).
This role will be aligned to support all the PCN services such as but not limited to Enhanced Health in Care Homes, Early Cancer Care,Mental Health, Learning disabilities, Digital and Enhanced Access. You may be required totravel to our member practices at times; therefore, the post holder would needto be a car driver and have access to a car.
Main duties of the jobAttend and minute the PCN monthly meetings circulatingminutes to all attendees, once approved by the meeting chair.
Assist the Clinical Director with any fact finding required
Support social media and website updates
Assist the PCN with the induction of new staff
Ordering of stationery and medical equipment and uniformswhere needed
Report back any issues with visits or staff, etc
Support with the organisation and clinics ofthe enhanced access service
Submit patient surveys for all PCN services.
Add clinics to Emis
Participating in the administrative and professional responsibilities of the PCN team
Ensuring accurate and legible notes of all consultations and treatments are recorded in patients notes
Data entry and coding of patient information pertaining to practice PCN Des & QOF requirements
Any other duties as required by the PCN

Park Lane Practice, Southlands Medical Group, Deerness Park Medical Practice, New City Medical Group, Ashburn Surgery, Riverview Surgery and Villette Surgery.
Sunderland East PCN works closely with the wider locality to improve and enhance patient care. Sunderland East PCN is committed to delivering an integrated neighbourhood team approach within the community and works to improve patient access to local services.

Person Specification Qualifications- Ability to develop and maintain good working relationships and communicate with a variety of audiences in a professional manner.
- Experience working across several locations, autonomously and as part of a wider team.
- Ability to demonstrate effective organisational skills and knowledge of working to time-plans.
- Intermediate knowledge of Microsoft Office packages including Word, Outlook, PowerPoint, and Excel.
- Previous experience working in similar role with strict guidelines, organising meetings and other aspects of project work.
- Experience of supporting service improvement using data/digital platforms and negotiating with relative teams in the process.
- Flexibility in approach to tasks undertaken.
- Excellent written, verbal communication and interpersonal skills.
- Ability to use initiative, work independently and as part of a team.
- A car driver and have access to a car.
- Basic understanding of Emis web, including running reports, analysing, and manipulating data and working within appointment systems.
- Experience working in general practice/healthcare organisation.
