We are looking for a friendly, professional, caring and organised person to join our team at Pen-Y-Bont. If you have excellent communication skills, a welcoming attitude and the ability to multi task in a fast paced environment, and able to work 25 hours a week, we would love to hear from you!.

Main duties of the job

The Patient Care Navigator/ administrator will be required to undertake various clerical duties associated with patient care and the efficient running of the GP practice, acting as a focal point for signposting and communication between patients, doctors, nurses, other members of the primary health care team and external services.

Some of the duties of the role include booking appointments, answering and making telephone calls, dealing with queries from patients, clinicians and other health professionals, prescription requests, photocopying, filing, scanning medical reports, processing new patient registration forms & undertaking general administration tasks.

The Practice is paper-light so the individual will be required to have good IT skills and good telephony skills.

The Practice consists of 4 GP Partners, 1 ANP, 2 Practice Nurses, 1 HCA and an excellent team of Care Navigators/ Administrators.

Job responsibilities
  • Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
  • Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone
  • To have a thorough knowledge of all practice procedures.
  • To work in accordance of written protocols/policies
  • Filing post in medical records
  • Photocopy/Scan documents as requested
  • Process appointment requests, via telephone and digitally.
  • Processing registrations of new patients.
  • Signposting to other services
  • Process patients change of address
  • Process repeat prescription request in accordance with practice guidelines.
  • Have working knowledge of telephone system.
  • Answer telephone where necessary
Person Specification Qualifications
  • 1. Good Standard of education
  • 2. Practical experience of working with others
  • 3. Experience of using own initiative / multi tasking
  • 4. Experience of excellent Customer Service
  • 5. Practical experience of computerised systems

Experience
