About the Role

The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service.

The main duties of the role include:

  • Working with GPs and other professionals within the practice to identify and manage a caseload of patients, particularly working with vulnerable patient groups with long‑term conditions that may require this extra support. This may include working with individuals, their carers, families and healthcare teams.
  • Using in‑depth knowledge of locally available support and services to develop care plans with patients, focusing on prevention.
  • Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient‑centred.
  • Helping patients prepare for, or follow‑up after, clinical conversations.
  • Supporting digital initiatives.
  • Using clinical systems on a daily basis, with confidence to run searches and keep care records up‑to‑date.
