Community Staff Nurse

Responsibilities

• Provide evidence‑based care as a Chronic Condition Case Manager to individuals with highly complex needs, through disease‑specific training.
• Participate in a modular training programme to acquire the clinical skills required for a Nurse Practitioner role.
• Deliver clinically effective care within the District Nurse Team.
• Travel between patients’ homes in towns and rural areas, maintaining timely service delivery.
• Maintain patients in their own homes and communities, reducing hospital admissions and empowering self‑management.

• Registered nurse, Band 5 Community Nurse, with experience in district nursing.
• Excellent communication skills.
• Willingness to undertake further training and develop into a Chronic Condition Case Manager or Nurse Practitioner.

You must be able to travel between patients' homes within towns and rural areas in a timely manner.

The ability to speak Welsh is desirable;
