Betsi Cadwaladr University Health Board is seeking a Chronic Condition Case Manager in Llangefni, Wales. The successful candidate will deliver evidence-based care to individuals with complex needs while participating in a training program for Nurse Practitioner roles. Candidates should be Registered Nurses with district nursing experience and excellent communication skills. Ability to travel between patients' homes is essential. Proficiency in Welsh is desirable but not mandatory.
