Community Nurse – District​/Home Care FTC

Job Overview

Fixed term/secondment for 6 months until 30.01.2027 due to maternity cover.

We are recruiting highly motivated, passionate Band 5 Community Nurses who are keen to develop and work towards one of the following roles:

• Band 6 District Nurse by undertaking the District Nurse Qualification
• Chronic Condition Case Manager by undertaking disease‑specific training to provide evidence‑based care for individuals with highly complex needs
• Nurse Practitioner by undertaking a modular approach to attain the required clinical skills

• Provide clinically effective care within the District Nurse Team and maintain high standards of care
• Communicate effectively with patients, families and multidisciplinary team members
• Travel between patient homes within towns and rural areas in a timely manner
• Support patients to remain in their own homes and communities, thereby averting hospital admissions
• Implement patient‑centred care and treatment aligned with evidence‑based practice
• Speak Welsh is desirable but not required;
  Welsh or English speakers are welcome

• Registered nurse or within 2 months of gaining PIN
• Knowledge of Primary Care

• Effective interpersonal and communication skills
• Ability to maintain high standards of care
• Clinical competency

• Flexible approach to work
• Welsh speaker

• Evidence of ongoing professional training
• Knowledge of professional codes of conduct
• Broad knowledge of relevant community nursing skills and competencies

• Meets all desirable criteria as per attached job description/person specification

• Working unsociable hours
• Ability to travel between patient homes in a timely manner
