Pharmacist in Continuing Care

As a key player in this position, you’ll provide essential clinical services while fulfilling contractual obligations. Your responsibilities will include managing disease states, performing medication reviews, and collaborating with other healthcare professionals to maintain high standards of patient care.

Key Responsibilities:

• Fulfill patient-centered care contracts and ensure compliance

• Conduct biannual medication reviews in facility committees

• Educate nursing staff on essential medication topics

• Prioritize safety and efficiency in pharmacy workflows

• Prepare quarterly reports and conduct audits on medication practices

Requirements:

• Bachelor’s degree in Pharmacy from a recognized institution

• Current Pharmacist license in good standing

• Certification in Injection and First Aid is mandatory

• Previous experience with Kroll software is an asset

• Additional certifications like Certified Geriatric Pharmacist preferred
